- Facility gives notice to current provider
- Administrator provides hire/no hire input on existing staff
- Administrator provides Consonus business cards to existing staff who wish to contact Consonus
- Area Director assigned and introduced to facility staff
- As soon as current provider communicates with current staff, RVP/AD meet with existing staff
- Recruiting develops potential employee contact list from licensee database Post all open positions and actively recruit for positions not likely to be filled by existing staff
- Order all hardware and have IT work with facility IT on internet connection

- Host recruiting event for current/potential staff. Invite facility management if desired
- Personal follow up phone calls to all attendees, complete offers
- Determine strategy for facility EMR to Rehab Optima integration

- AD Reviews payer contracts with business office
- Schedule transition meeting

- Transition Meeting
- AD attends UR meeting

- AD meets with MDS coordinator to review PPS patients on caseload
- Enter current patients into Rehab Optima (or finalize date for integration)
- Complete transition clinical documents for carryover of plan of care
- Host team dinner/event for new team
- Complete therapist/patient schedules for day 1
- Staff completes mandatory training webinars

- Rehab team “Open Gym” to meet and greet facility staff and residents
- AD attends meetings with RD
- Schedule Clinical Director trainings in programs and documentation

- AD assists with end of month close out and invoice preparation
- All operational processes in place
- Offer 6 month business review to ensure mutual agreement on strategic progress
